Cordova Community Medical Center’s Long Term Care offers skilled nursing care to loved ones needing short term rehabilitation or more complex long-term care. Residents are welcomed by the staff – often familiar faces who have established a provider-based relationship long before an individual needs long term care.
We put a tailored plan of care into place for each resident upon admission. Family members are included in designing the best plan, which considers the resident’s life-style, medical needs, and social preferences. Personal choice is always encouraged and residents are offered options for their activities, both in and out of the facility.
Families face difficult decision when dealing with a loved one who needs a higher level of care beyond what the family can provide. An in-house social worker is available to help families through this transition.
Our residents’ medical needs are met by a professional, loving, licensed staff twenty-four hours a day, seven days a week.
Compassionate and caring nursing assistants provide for the daily needs of each resident. Medical staff coordinate multidisciplinary care conference reviews, conduct diabetic monitoring, and offer other patient-focused services to ensure each resident receives the level and type of care they require.
We work together to make this a home away from home. Some of the activities our residents enjoy include:
- Resident / family picnics
- Outings to various activities (fishing, berry-picking, animal-watching, sporting events)
- Church services
- Greenhouse and garden
- Accessible grounds, and patio with a pond, barbecue and flower beds
