Please follow the instructions below for determining if you need a COVID-19 test and how to register.
Step 1: Do You Need a Test?
Asymptomatic: If any of the below bullet items apply to you, then you need a test.
- If you recently traveled and require the initial arrival or 7-10 day repeat test
- You need a test prior to a medical procedure
- You work for critical infrastructure and your employer has a prearranged plan with CCMC to provide testing.
Symptoms: If you think you have COVID-19 symptoms, please call one of our healthcare providers for evaluation.
After evaluation, if COVID-19 testing is ordered, you can preregister for your test and come to pre-scheduled testing time.
Step 2: Register for COVID-19 Test
COVID-19 Testing is available at prearranged advertised hours in the ambulance bay. Current hours are Tuesdays and Thursdays, from 9am – 10am. Please pre-register by filling out the form below and submit with an image of Insurance Card and Drivers License.
